The Department of Human Services, Division of Aging Services supports older adults, people with disabilities, and caregivers through a variety of services, such as delivering meals to homes, providing legal assistance and offering Medicare counseling. What is the Community Care Services Program (CCSP)?
The CCSP helps eligible consumers avoid placement in a nursing facility by providing social, health and support services in their homes and communities. The CCSP is a Medicaid waiver program suitable for individuals who meet financial eligibility requirements, secure a doctor's approval, and are considered at-risk for nursing home placement.
CCSP-covered services include adult day health; emergency response services; home-delivered meals; alternative living services; and personal support services such as housekeeping, meal preparation, errands, and caregiver respite.
