A new federal highway safety initiative requires all Commercial Driver’s License (CDL) holders to provide information to the Department of Driver Services (DDS) regarding the type of commercial motor vehicle operation they drive, or expect to drive, with their CDL.

According to Federal Medical Certification Requirements (§ 383.71(b)(1) Code of Federal Regulations 205, drivers operating in certain types of commerce must submit a current Department of Transportation medical examiner’s certificate/card to DDS to get a certified medical status as part of their driving record. If you're required to have a certified medical status and don't provide or update your DOT medical information with DDS, you will not be certified and may lose your commercial driving privileges.

Almost 328,000 Georgia CDL holders rely on commercial driving privileges for their livelihood, and it is a vital component of our state commerce.  In January 2012, DDS began urging CDL holders and their employers to be proactive in providing this information.  The Federal Motor Carrier Safety Administration has set a compliance date of January 30, 2014.

The Process

DDS has expanded its online services to help commercial drivers self-certify their type of commerce as federal rules require and, if needed, securely upload medical certification documentation in a faster and more convenient way.  By visiting the DDS website and creating an account, you will have 24/7 access to update and/or transmit documentation whether at home or out on the road.

You may also fax a copy of the self-certification form and medical certificate/medical waiver (if required) and notice received from DDS (if applicable) to (770) 918-6251. You  can also mail documentation for multiple customers. Please include copies of each medical certificate/medical waiver (if required) and notice (if applicable) and self-certification form to:
