What is the Community Care Services Program (CCSP)?

The CCSP helps eligible consumers avoid placement in a nursing facility by providing social, health and support services in their homes and communities. The CCSP is a Medicaid waiver program suitable for individuals who meet financial eligibility requirements, secure a doctor's approval, and are considered at-risk for nursing home placement.

CCSP-covered services include adult day health; emergency response services; home-delivered meals; alternative living services; and personal support services such as housekeeping, meal preparation, errands, and caregiver respite. Learn more about these services. In addition, a wide range of non-Medicaid services are available to consumers who may not qualify for the CCSP.
