• Make Sure You're Eligible

    To qualify, you must be eligible for Medicaid and:

    • Be capable of managing your own services. 
    • Be medically stable but at risk of placement in a hospital or nursing facility.
    • Be safely placed in a home or community setting.
    • Other factors help determine eligibility; length of time on the waiting list, ability to live independently, and the estimated cost of care.

  • Gather What You'll Need

    While no documents are required, you will need to provide information about your medical history.

  • Submit Your Application

    Apply through Alliant Health Solutions, a nonprofit organization.

    • Apply by Phone
      • Call Alliant Health Solutions at 888-669-7195.
      • When prompted, select the ICWP option on the telephone menu.
      • Once connected, an Alliant Health Solutions staff member will screen you by asking questions about your situation.
      • If the screening requirements are met, Alliant Health Solutions will have you submit an application.
      • After submitting your application, you will schedule an assessment.

  • Next Steps

    • Once approved, you’ll receive services as funding becomes available.
    • Participants, their families, case managers, and providers will work together to create a plan of care.
