Request a Criminal Background Check?

There are 2 ways to request someone else’s criminal history record: with or without their consent:
- To request a criminal history with the individual’s consent, you must first obtain written and notarized consent from the individual, along with a copy of their legal ID. The form must include the person’s name, race, sex, and date of birth.
- If you already have the individual’s full name, race, sex, and date of birth, you may also request a criminal history record without their consent.

Gather What You’ll Need
- Your government-issued identification (valid driver’s license or state ID card, passport, military ID, immigration card, or birth certificate)
- Signed, notarized consent of the person whose criminal history you are requesting (if requesting with their consent)
- Copy of government-issued identification of the person whose criminal history you are requesting (if requesting with their consent)
- A valid form of payment (fees and payment methods will vary by county)
