Certain immunizations and screenings are required to enroll in school.

Children attending any childcare facility, pre-kindergarten, Head Start program, nursery, or school in Georgia are required to have certain forms on file. The Georgia Department of Health maintains a list of required immunizations and health screenings for children. All completed forms must be submitted upon admittance or enrollment.

  • Get Prepared

    Get your child properly vaccinated. Some vaccinations require multiple doses at specific times, so please plan ahead to get all your required doses administered. Contact your local health department or physician to set up appointments to get these vaccinations. See summary of immunization requirements for child care and school attendance.

  • Gather What You'll Need

    • Immunization Certificate: You must submit proof of required vaccines with an Immunization Certificate (Form 3231). Your local health department or physician can complete the form for you. If your child has received vaccines in another state, you may have to bring a copy of your child’s immunization records from that state in order to complete the certificate.
    • Certificate of Vision, Hearing, Dental, and Nutrition Screening: All children enrolled in a pre-kindergarten program or public school must have a Certificate of Vision, Hearing, Dental, and Nutrition Screening (Form 3300). Your local health department or physician can conduct these screenings and complete this form.
    • Exemptions: If you are claiming a medical exemption for a specific vaccine, the medical exemption must be marked on the Immunization Certificate. A letter from a physician will not be accepted as a medical exemption. If you are claiming a religious objection, you must sign and submit a notarized Affidavit of Religious Objection to Immunization (Form 2208) to the school or child care program. Please be aware that a child with medical or religious exemptions may be excluded from attending the school or childcare facility.
  • Submit Required Forms

    Children attending any childcare facility, pre-kindergarten, Head Start program, nursery, or school in Georgia are required to have certain forms on file. Schools may grant an extension waiver of up to 90 days in order to allow parents time to submit all required documents.
